What should I enter in a record?
Copy the vaccine name, brand, dose, and administration date from your child’s vaccination card. You can also record the provider, clinic, and notes from the visit.
Where does the next-dose date come from?
Enter the next appointment date given by your child’s clinician. This preview does not calculate or recommend a medical schedule.
Can I attach the vaccination card?
Yes. On the last page, select up to four JPG, PNG, WebP, or PDF files, no larger than 5 MB each. Image previews are available before saving. This demo keeps only document names in the completed record; it does not upload files.
Are my records saved permanently?
This is a static UI preview. Your new entries and reminder choices remain available while navigating these pages, but reset on refresh. No medical records are sent to a server.
Can I share a copy?
Open Download records to print the summary, save it as PDF using your browser, or download a CSV. The exported summary is marked as demo data.
